Why is the NHS waiting list so long?
More people need hospital care than the NHS has the capacity to treat on time. The list was already growing before the pandemic, then jumped when operations were postponed. Too few staff, beds and operating theatres, and delays getting people out of hospital, stop the backlog clearing.
Why it happens
- More people need care. An older population has more long-term conditions, so demand grows every year, faster than the NHS has grown.
- The pandemic backlog. Millions of operations and appointments were postponed in 2020 and 2021, and the waiting list rose from 4.6 million to over 7 million.
- Not enough staff and capacity. There are vacancies in nursing and medicine, and fewer hospital beds per person than in similar countries. Strikes in 2023 and 2024 caused more cancellations.
- Patients who cannot leave hospital. Thousands of beds are taken by people who are well enough to go home but need social care or support that is not available, so new patients cannot be admitted.
